You MUST let us know and RETURN ALL FORMS PLEASE. We have only had 100 returned.
The IMPORTANT thing to REMEBER is that the local NHS team need to KNOW WHETHER you are saying YES or NO regarding FLU VACCINATION - either way, it it important the NHS TEAM knows.
Please return the NHS FORM ASAP.
